PDE 6004, Act 24, 34, 114, 151 Clearances (PLEASE GO THROUGH COMPLY)

Please print and read this form and submit the signature page only to the District Office. Act 24 requires all current school employees and volunteers to complete a Pennsylvania Department of Education (“PDE") form to report prior arrests or convictions for any offense listed in the newly expanded Section 111(e).  If you have committed any of the reportable offenses listed on the 6004 form since your last clearance date, you have 72 hours to submit a new 6004 form.

Criminal History Clearance (Act 34) Please complete and pay for the online form ($22) 

Child Abuse Clearance (Act 151) You must provide a copy of the Pennsylvania Child Abuse History Clearance from the Department of Human Services (formerly Dept. of Public Welfare). Check the appropriate box: "School Employment" block. ($13)

 

Act 114 Fingerprinting Application The Service Code for PAID employees is 1KG6XN.  Registration and payment are done online. Take the registration page with you to get your fingerprints taken at an authorized IdentoGO fingerprint location. By sending the District Office a copy of your FBI registration page, we can check the results online. You will be mailed a letter from the FBI with the results. ($25).
